Sione Teo, aged 65, was last seen at his home on Hartley Valley Road, Vale of Clwydd about 1.30pm on Thursday, January 28.
Sione is described as being of Pacific Islander appearance, with short grey and black hair, and a black moustache.
He was last seen wearing grey tracksuit pants and a blue and white coloured long sleeve shirt.
Following inquiries, it's believed Sione may have been in the Blacktown area earlier today (Friday, January 29).
Family and police hold serious concerns for his welfare due to medical conditions, and urge anyone who sees Sione, to come forward.
