Two people, a man and woman in their 70’s, suffered serious injuries and were airlifted from the scene to Westmead and Liverpool hospitals.
The occupant of a second vehicle involved in the crash was transported by road to Lithgow Hospital in a stable condition with minor injuries.
The crash occurred at about 12.15pm on the Great Western Highway. Paramedics worked alongside doctors who landed on the road near the accident site to stabilise the two elderly people before they could be airlifted.
Six people were taken to hospital after a separate crash that occurred just 45 minutes later on the Bells Line of Road.
The three-vehicle crash at Bilpin just after 1pm on Saturday involved a collision between a Toyota Camry, Holden Cruz and Subaru Impreza.
The Toyota had been travelling east on Bells Line of Road, while the Cruz and Impreza were travelling west.
The 79-year-old man driving the Holden and his 72-year-old female passenger were treated at the scene before being conveyed to Westmead Hospital in a stable condition.
The 18-year-old woman driving the Toyota and the 17-year-old boy in the front seat were treated at the scene and before being taken to Westmead Hospital in a stable condition.

The rear seat passenger, a 15-year-old boy, was airlifted to Liverpool Hospital in a critical but stable condition with internal injuries.
The 23-year-old male driver of the Subaru was uninjured but was taken to Hawkesbury Hospital for mandatory blood and urine tests. His male passenger was also uninjured.
Officers from Hawkesbury Police District attended and established a crime scene. The road was closed for several hours while specialist police examined the crash scene.
Crash Investigation Unit officers are investigating the circumstances surrounding the incident.
